 ViCare integrates this process with the primary care physician and the clinical team to provide each of the care team members and the patient with a clear, consistent, and coordinated program and to assure the participant of confidentiality. 

The ViCare Guide engages the patient either face-to-face or virtually through a HIPAA-secure telehealth system.


 Eligible individuals are identified from an employer’s health plan claims data. One-on-one coaching is enhanced through the HIPAA cloud platform enabling electronic patient reminders, educational content, and care coordination.
